1. Acknowledgment of Risk
I acknowledge that participation in soccer-related activities, training sessions, and events (“Activities”) organized by Palestino Soccer Academy involves inherent risks of injury. These risks may include, but are not limited to, sprains, fractures, concussions, muscle strains, and other serious injuries. I voluntarily choose to participate with full awareness and understanding of these risks.
2. Assumption of Risk
I knowingly and willingly assume all risks associated with participation in the Activities. I understand that my participation is voluntary and that I am not being pressured or forced to take part in any academy-related activities.
3. Release of Liability
In consideration of being allowed to participate, I hereby release, indemnify, and hold harmless Palestino Soccer Academy, including its coaches, staff, employees, volunteers, agents, and affiliates, from any and all claims, liabilities, damages, losses, or injuries arising out of my participation in the Activities. This release includes claims resulting from the negligence of the academy, its representatives, or other participants.
4. Emergency Medical Treatment
I authorize Palestino Soccer Academy staff to obtain emergency medical treatment for me (or the participant if under 18 years of age) in the event of injury or medical emergency. I understand that I am solely responsible for all medical expenses incurred and release the academy from any financial responsibility related to such treatment.
5. Fitness to Participate
I confirm that I am physically fit and capable of participating in soccer activities. I agree to disclose any medical conditions, injuries, or physical limitations that may affect participation. I understand that it is my responsibility to consult a medical professional if I have concerns regarding my fitness to participate.
6. Parental / Guardian Consent (If Applicable)
If the participant is under 18 years of age, I, as the parent or legal guardian, consent to their participation in the Activities. I agree to all terms of this waiver on behalf of the minor and accept responsibility for their well-being during participation.
7. Media Release
I grant permission to Palestino Soccer Academy to use photographs, videos, or other media taken during Activities for promotional and marketing purposes. This may include use on the academy’s website, social media platforms, and printed materials.
8. Governing Law
This Liability Waiver Agreement shall be governed by and interpreted in accordance with the laws of the State of New Jersey. Any disputes shall be resolved in the appropriate legal venue located in Woodland Park, New Jersey.
9. Severability
If any provision of this agreement is found to be invalid or unenforceable, the remaining provisions shall continue in full force and effect.
